Hypertension, thought to afflict more than 50 million Americans, is a syndrome with multiple causes; but it is often both a cause and co-morbid factor in heart disease and, so, deserves mention here. Hypertension is generally defined as an arterial pressure of greater than 140 90 mmHg for an extended period of time. Untreated, it can lead to renal disease and stroke, as well as cardiac disease. Patients with uncontrolled hypertension should seek medical care prior to treatment whenever possible. And elective treatment should be deferred for patients with severe hypertension. Judicious use of epinephrine is appropriate for those who are under treatment and or with mildly elevated pressures Table 6 ; . Regarding the phrase "judicious use of epinephrine, " the joint conference of the American Heart Association and the American Dental Association noted, "the typical concentrations of vasoconstrictors contained in local anesthetics are not contraindicated with cardiovascular disease so long as preliminary aspiration is practiced, the agent is injected slowly, and the smallest effective dose is administered."7 It should be understood that the exact amount of epinephrine that is safe to use on the "cardiac" dental patient is dependent upon the specific type and degree of cardiac disease present, as well as the patient's weight and age. However, it is generally accepted that the total dosage of epinephrine be limited to 0.04 mg within a 15minute period in cardiac risk patients.3, 4 This equates to approximately two cartridges of 1: 100, 000 epinephrine-containing local anesthetic. Levonordefrin neo-cobefrin ; is considered to be roughly one-fifth as effective a vasoconstrictor as epinephrine and is, therefore, used in a 1: 20, 000 concentration and should be considered to carry the same clinical risks as 1: 100, 000 epinephrine.4, 7.

Table 2 lists common angiogenic skin disorders and therapies that have been used to treat them. The inhibition or stimulation of angiogenesis may contribute to the efficacy of these drugs.10, 12-24 Modulating angiogenesis is only one of many mechanisms by which drugs may affect the dermatologic diseases listed in our survey. Comparisons with clinically proven therapies have shown that 20 percent AzA cream is an effective monotherapy in mild to moderate forms of acne, with an overall efficacy comparable to that of tretinoin 0.05 percent ; , benzoyl peroxide 5 percent ; , and topical erythromycin 2 percent ; . Prudent Use of Derivatives is Widespread and Compelling The derivatives market has ballooned in size over the last decade; in fact, the cash markets now often take their cue directly from the synthetic, or derivatives, markets. Here at ING we use derivatives as a substitute for cash securities that we would otherwise purchase. Whether derivatives are used to establish interest rate exposures such as duration targets or the term structure of a portfolio or to buy or hedge credit risk, the synthetic markets are the most efficient and expeditious means to re-balance a fixed income portfolio. Reducing transaction costs and increasing the precision of portfolio construction logically leads to better portfolio performance for the same degree of risk. More return for less risk is, of course, the Holy Grail of investment management. Understanding, Monitoring, and Measuring Risk is Critical Behind every headline about a derivatives crack-up is an untold story of risk misunderstood, ignored, or misrepresented. Our fixed income Risk Management Group employs several proprietary systems to monitor the effects of derivatives instruments on the portfolio. As with any financial instrument purchased for the account, one of the top measurements is the instrument's effect on duration for the entire portfolio. We carefully monitor the impact on the portfolio's overall duration as this is a primary driver of our investment decision making process. ING has developed a proprietary risk management platform to aggregate exposures and forecast risk for all of our fixed income client's portfolios. The Risk Management group has a rigorous monitoring process in place to ensure, on a daily basis, that each portfolio is in line with the strategic targets for the relevant investment style. Targets are set along sectors and maturity based on market weights and exposures. Portfolios within each investment style are aggregated by sector and maturity to allow for easy monitoring of adherence to targets. Tolerance excesses are communicated to the relevant portfolio manager and the head of third party fixed income along with the required trade amounts to bring the portfolio back into line.
