HDK is one of the most active agents used in the treatment of PC and has an incredibly broad spectrum of pharmacologic activity. In addition, it has the potential to be synergistic with chemotherapy compounds and or block the enzymatic degradation of multiple anti-cancer agents. HDK blood level monitoring is an excellent tool to evaluate absorption and hence bioavailability of this anticancer agent. These unique properties are of great value in the therapy of prostate cancer. Unfortunately, this agent has never been approved by the FDA for the treatment of PC. Many physicians are unaware of the efficacy of HDK or are afraid of its toxicity based on exaggerations of HDK's effect on the liver. Certainly, HDK should be considered for active therapy of PC and be evaluated in welldesigned trials that take advantage of our better understanding of the pharmacology of anti-neoplastic agents. While this is being done, clinical trials exploring structurally similar agents that can be given less often, or that might have better bioavailability without issues of gastric acidity, should be undertaken. HDK + HC is one of the most active regimens in the therapy of PC, because benazepril and hydrochlorothiazide. If it is answered, "yes " to any one of these three questions, then there is cause for concern that the individual may have BDD and be referred to a mental health professional. Since the 1995 Norman Hugo appellate decision, it is incumbent that surgeons screen for this possibility. This presentation will impact the forensic community and or humanity by demonstrating how disorder with a high rate of depression, suicidal ideation, and suicide attempts. Most informed consent for surgeries that modify body image such as cosmetic, dermatological, bariatric procedures that add three questions that would service both the surgeon's informed consent requirement and screen for a serious comorbid disorder, BDD with prompt referral for mental health assessment and treatment. Body Dysmorphic Disorder BDD ; is an uncommonly discussed psychiatric illness in cosmetic, bariatric, dermatological surgical settings. For patients who are seeking such procedures, the BDD diagnosis can be more easily ascertained if three questions are added to the informed consent document: 1 ; does the individual spend excessive time concerned over this defect to the point where it interferes with normal activities on the job, school or home?; 2 ; is the defect exaggerated and in reality only minimal in appearance to most people?; 3 ; does this individual constantly checks the defect and experience emotional pain, and avoids mirrors or deliberately camouflages the defect? If the patient answers "yes" to one of these questions he may have BDD and a referral to a mental health professional should be considered for further interview, psycho education, psychotherapy Cognitive Behavioral Therapy ; , and possible pharmacotherapy. The well-publicized case of Dr. Norman Hugo leads to an appellate decision in 1995 stimulated professional attention to a formerly less discussed disorder. The original contention of the malpractice lawsuit. In practice, there exist several prediction schemes that could be used for this purpose. The predictive filtering methods discussed in Section 3.1.1 are suitable candidates. For example, in [Tan97], Tanskanen et al. proposed a polynomial predictive filter-based power regulation. This method is based on the assumption that the fading signal is a piecewise low-degree polynomial. The future values of the power signals are predicted by an H-N FIR predictor of length 15. Simulations demonstrate that the peaks in the power signals are smoothened. In various cases with different numbers of multi-users and mobile speeds, the channel output variance is however reduced only by 2%--3%. This marginal improvement is due to the one-bit command transmission bottleneck. We have presented three power control strategies for mobile communications systems. Comparisons of these power regulation methods are given in Table 3.2. Generally, the advanced controllers can provide better regulation characteristics than the conventional methods, e.g., `bangbang' control. Nevertheless, they often have the drawback of heavy computational complexity. Additionally, the limiting single-bit power command transmission mode is not usually taken into consideration in the design of advanced power regulation schemes. Consequently, the command transmission mode has severe negative effects on the performance of these power regulation schemes. This shows that it is not practical to develop advanced control schemes, if the power command transmission bottleneck is not first relieved. In [Cha95], Chang and Wang proposed a variable step size approach to cope with this problem, which needs more channel capacity. We tackle the problem by introducing the fuzzy logic-based power command enhancement unit at the mobile station in publication [P3]. It will be discussed in the following chapter.
