If the generic drug msrp called awp, average wholesale price ; isn't significant different than the branded drug's awp, then it's possible that the insurance company gets a better deal on the branded vs generic and is thus able to offer a lower copay on the branded.

Tion by a serum component. However, this seems unlikely, as we have shown that serum and biologically active compounds contained therein are not required for the transcriptional activation of Mel 1a mRNA Fig. 8 ; . Thus, to account for the increase in Mel la mRNA levels in the absence of serum, one can propose two potential autoregulatory or feedback mechanisms. The first possibility is the Mel la mRNA is under a constitutive expression that can be up-regulated by the lack of receptor protein at the cell membrane or in the absence of melatonin. A second possibility is that the PT secretes a product that is able to act in a positive manner on the PT cell to up-regulate the transcription of the Mel 1a mRNA. In both cases melatonin can reduce the up-regulation to basal levels of expression. In summary, this study provides the first indication of the dynamic regulation of melatonin receptor mRNA and protein in the ovine PT and provides the first evidence of the mechanism of this regulation, which. CORTICAL PHOTOTHROMBOTIC ISCHEMIC LESION: INFLUENCE OF HYPOXIC PRECONDITIONING AND MELATONIN APPLICATION I. Matjovsk, K. Bernskov, J. Mares, D. Krsl Department of Normal, Pathological and Clinical Physiology, 3rd Medical School, Charles University, Prague Rose Bengal RB ; technique of photothrombotic occlusion of blood vessels is a commonly used model of focal ischemic lesions. It is based on the sensitization of endothelial cells and the subsequent activation of blood coagulation, which are both closely related to the formation of reactive oxygen species ROS ; . ROS are also involved in the general mechanisms of neural injury and hypoxic ischemic preconditioning. We tested the sensitivity of this model of focal ischemia to preconditioning with hypobaric hypoxia and pre-treatment with the ROS scavenger melatonin. Experiments were performed on adult male Wistar rats. Controls were not exposed to any other experimental manipulation. One group of experimental animals was exposed to hypobaric hypoxia 1 hour, 9 000 m ; 72 hours before irradiation. Other group was pre-treated by i.p. melatonin 1 hour before photothrombosis. Under ketamine xylazine anesthesia the RB solution was applied via the tail vein. Immediately after application the scull over the left sensomotor cortex was exposed to a laser beam 532 nm ; for 9 min. 72 hours after the irradiation, the brains were perfused, 1 mm coronary slices were obtained and stained with TTC 2, 3, 5-triphenyltetrazolium chloride ; . In the control group, surface lesion was observed only in 1 animal, while 15 animals developed deep damage involving the striatum. In the group pre-exposed to hypoxia, 1 animal displayed a surface defect and 10 animals showed severe lesions. In the group of animals pre-treated with melatonin, surface lesion was observed in 10 animals, whereas only 4 animals developed deep lesion. Chi square test has shown highly significant differences among all three groups. Conclusions: Preexposure to hypobaric hypoxia did not induce significant protection of ischemic damage in our experiment. However, pre-treatment with melatoinn significantly reduced the size of brain tissue damage in this model. We suppose that the scavenger effect of melatonih influenced primarily the pathological changes in the vascular endothelium and the subsequent blood coagulation during and after irradiation. In May 2000, the Maine legislature passed the Maine Rx Program, which allows the state to negotiate fairer drug prices for all residents, regardless of income level or age, by using the buying power of its Medicaid program. Maine Rx also gives the state the authority to establish maximum retail prices for pharmaceuticals if negotiations failed to lower drug prices within three years. The U.S. Supreme Court heard arguments in a lawsuit brought by the Pharmaceutical Research and Manufacturers Association PhRMA ; , ruling in May 2003 that the program does not interfere with interstate commerce and that the state could go forward with implementing the program. Meanwhile, dozens of states are considering adopting programs similar to Maine Rx. Concerned over future legal challenges, Maine Governor John Baldacci signed legislation in June 2003 amending the state program to limit its benefits to Maine residents whose income falls under 350% of the federal poverty level $64, 400 for a family of four and $31, 400 for an individual ; and to individuals whose drug expenses exceed 5% of their income and naprelan. 3.
