Paecilomyces lilacinus, an uncommon cause of exogenous endophthalmitis, presents a challenging management problem since the organism is usually resistant to amphotericin B, the most commonly employed intravitreal antifungal, and natamycin, a frequently employed topical antifungal. The organism is sensitive to miconazole nitrate, ketoconazole, and, in some cases, fluconazole. We report 4 cases of endophthalmitis caused by P lilacinus and discuss treatment options. Report of Cases. Case 1. A 37-yearold man with hand motion vision in the left eye underwent penetrating keratoplasty, extracapsular cataract extraction, and posterior chamber intraocular lens implantation in the left eye 4 years after a bilateral ammonia chemical injury that resulted in persistent keratitis and corneal thinning. One day postkeratoplasty, he underwent wound leak repair. Examination 3 weeks later demonstrated a best-corrected visual acuity of 2 200 OS and a white flocculent mass on the cornea extending into the anterior chamber Figure 1 ; . After aqueous and vitreous specimens demonstrated filamentous fungi Figure 2 ; , a total of 4 amphotericin B injections 5-10 g 0.1 mL ; were administered 3 into the anterior chamber and 1 into the vitreous ; . Due to enlargement of the flocculent mass the patient underwent partial iridectomy, removal of the mass, and a fifth anterior chamber injection of amphotericin B. About a reassessment ago a finding persistent that miconazole is pointlessly nothing like it. Experience on the drug line was that inadequate application to all mucosal surfaces was leading to higher than expected failure rates in eradicating oral thrush in babies, resulting in ongoing symptoms for mothers and babies and often the need to use oral fluconazole. Although Hoppe used the equivalent of 1ml in his trial, this would presumably have involved close supervision of effective application by researchers implementing the protocol which may be less relevant in clinical practice. This may explain the lack of benefit from small quantities. Quantity of gel to be used It was decided that 5ml four times a day should be the suggested amount, on the assumption that a portion of this would be lost; either dribbled down mum's hand or the baby's chin. By defining an amount to be squeezed out of the tube, we hoped that mothers would apply enough gel to coat between the baby's cheeks and gums, and the roof of the hard palate as well as the tongue. The amount needed will vary with the age of the baby and should be applied slowly and gently to ensure adequate coverage of oral mucosa and not just the tongue. Frequency of application By applying the gel four times a day, Hoppe et al 1997 ; produced excellent cure rates with few adverse effects.
